CANVAS (CANcer surVeillance And Support) looks at the use of an internet-based/online symptom monitoring tool that allows you to report any symptoms to your hospital team after you have completed your treatment. This new system is being implemented in addition to your standard follow-up and taking part will not affect the usual care you receive from your hospital team. The CANVAS system is not a replacement for your usual care.

We would like you to complete the symptom report every 4 months whether you are experiencing symptoms or not. You will receive a text/email reminder prior to your completion being due.

You can complete more frequently if you feel unwell and want to receive advice on how to manage symptoms.
